Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description:

The Case Manager advocates for clients by providing specialized support and care coordination for individuals housed through Illumination Foundation. This role ensures that clients’ needs are addressed through comprehensive plans, resource linkage, and continuous support to foster stability

This roles pay range is $22-$25 per hour.
The schedule for this role will be Wednesday-Sunday: 9:30am -6:00pm. 

Ensure comprehensive case management is provided by Evaluating client’s need, developing
treatment plans, monitoring progress, aiding clients, and referrals, following up with weekly goals
and completion of action steps

  • Complete care plans and utilize Electronic Health Record(s) (EHR) and client databases (i.e., KIPU,
  • Homeless Management Information System (HMIS), Health plan programs) to track and maintain accurate case notes, client, and program data
  • Collaborate with the Case Management Team by attending weekly meetings to discuss the effectiveness of programs and client progress, and create new strategies to meet client’s level of need and improve treatment plans
  •  Link clients to resources to assist patients’ psycho-social and daily needs such as health care, nutritional needs, hygiene items, and referrals to other agencies for transitional/permanent supportive housing
  •  Respond to client crises, including behavioral or medical emergencies, by employing de-escalation techniques and coordinating with emergency services when necessary
  • Prepare and manage client-related documentation and correspondence
  • Establish and maintain confidential case files for participants and review required statistical reports for program management and evaluation.
  • Utilize IEHP portal to assist clients with accessing Insurance benefits and Authorizations

  • Go beyond in finding and linking unhoused clients with available resources needed.
  • Monitor clients’ progress towards goals and provide comprehensive services empowering clients towards self-sufficiency

  • Work with individuals who have backgrounds involving legal or justice system involvement
  • Working alongside other onsite/offsite departments
Required:
  • Bachelor’s degree in social services, Healthcare, or related field; or equivalent combination of training and experience.
  • Experience in homeless services, case management, and mental health support
  • Current valid CA driver's license
  • Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions.
  • At least 1 year of experience working with at risk/unhoused individuals (preferred)
  • Bilingual in Spanish and English (preferred)
  • Knowledge of resources available in corresponding counties Preferred:
  • Bilingual in English and Spanish.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Calendar, etc.)
