ICMS Case Manager

Homelessness is the largest social and public health crisis in California. Illumination Foundation(IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description

The ICMS Case Manager is responsible for assessing the strengths and needs of clients in the PSH (Permanent Supportive Housing) program. ICMS Case Manager is linking clients with appropriate resources, creating housing and service plans, monitoring clients’ progress towards goals established in service plans, empowering clients towards self- sufficiency, practicing active listening, providing trauma-informed care, and acknowledging and documenting client progress. The ICMS Case Manager also links clients to medical care, mental health care, alcohol/substance use treatment, financial support and other supportive services. This position requires an ability to maintain strict confidentiality and handle information with tact and diplomacy.

The pay range for this position is $23.00 - $26.00 per hour, depending on experience.

Responsibilities

• Maintain a caseload of 25+ individuals.
• Develop a service plan, evaluate the households’ needs, establish goals, and reassess households’ needs as needed.
• Conduct regular home visits, room checks, house meetings, and complete wellness checks for Micro-Communities & scattered site clients.
• Ensure clients are actively pursuing income opportunities through job searches, skill development, applying for benefits, or engaging in other income-generating activities.
• When attending appointments with clients, advocate for their access to stable housing, healthcare, education, and support for their social and emotional well-being.
• Coordinate and execute transportation for client appointments if deemed necessary.
• Ensure clients actively participate in life skills development, therapeutic interventions, and community engagement activities to foster personal growth.
• Ensure residency meets habitability standards by regularly inspecting for safety, cleanliness, and adhering to local housing regulations.
• Provide comprehensive support to households by identifying and connecting them with appropriate resources and other community-based providers.
• Ensure each household member is connected to medical insurance and linked to a medical care provider.
• Attend individual and team supervision sessions to review the program, organization, client progress, and develop new strategies to address client needs.
• Work cooperatively with Housing Specialist and other departments at The Illumination Foundation to ensure that clients successfully maintain permanent housing.
• Prepare clients for a move by addressing their emotional needs, ensuring they have the proper supplies, and providing regular check-ins.
• Assist clients with completing their annual recertification process, including gathering necessary documentation, submitting required forms, and ensuring compliance with program guidelines.
• Document client interactions in the Homeless Management Information System (HMIS) and internal Electronic Medical Records (EMR-KIPU), ensuring case notes and records are accurate and up to date.
• Uses a personal vehicle to conduct home visits to micro-communities, scattered sites, and fulfill other organizational transportation needs.
• Respond to phone calls and emails from clients, staff, and outside entities promptly.
• Maintain ethical and professional boundaries at all times, exercise sound judgment, and adhere to best practices when working with clients.

Preferred Experience/Minimum Qualifications

Required:

• Bachelor's degree in social services, Healthcare or related field; or 2 years equivalent combination of training and experience.
• Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
• Valid CA Driver’s License
• Eligible for company vehicle insurance.
• Possesses a reliable personal vehicle and maintain current automobile insurance.

Preferred:

• Bilingual in Spanish.
• Experience in the area of non-profit housing and/or housing for people with disabilities and chronic health conditions.
• At least 1 years of experience working with at risk/homeless individuals
• Experience with Management Information Systems (HMIS)
• Experience with KIPU(EMR)  Experience with housing first model, harm reduction, and de-escalation.
• Proficiency in Microsoft Office (Mail, Docs, Sheets, Calendar).
