Job Description
The ICMS Case Manager is responsible for assessing the strengths and needs of clients in the PSH (Permanent Supportive Housing) program. ICMS Case Manager is linking clients with appropriate resources, creating housing and service plans, monitoring clients’ progress towards goals established in service plans, empowering clients towards self- sufficiency, practicing active listening, providing trauma-informed care, and acknowledging and documenting client progress. The ICMS Case Manager also links clients to medical care, mental health care, alcohol/substance use treatment, financial support and other supportive services. This position requires an ability to maintain strict confidentiality and handle information with tact and diplomacy.
The pay range for this position is $23.00 - $26.00 per hour, depending on experience.
