Site Intake Coordinator

Homelessness is the largest social and public health crisis in California. Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
Role
The Site Intake Coordinator is responsible for completing client intakes at agency facilities and assisting case management staff with client care needs. A site intake coordinator is also integral in ensuring a supportive and secure environment for clients, addressing immediate daily needs, ensuring safety, and upholding cleanliness standards that promote the well-being of the residents.
The starting pay rate for this position is $20.00 per hour. This position is also eligible for $1.00 swing shift differential and $2.00 graveyard shift differential. The schedule for this position is Wednesday - Sunday, 1:00PM to 9:30PM.
Responsibilities
- Complete client intakes in a timely manner in accordance with agency workflows, policies, and documentation standards.
Maintain accurate and up-to-date client records within the agency’s Electronic Health Record (EHR) and HMIS systems.
Demonstrate a strong understanding of Medi-Cal and Medicare coverage and utilize this knowledge to connect clients with appropriate insurance-funded programs that address social determinants of health.
Orient clients to facility policies, amenities, expectations, and program guidelines upon intake.
Communicate effectively with the care team regarding client needs and assist with connecting clients to urgent or necessary resources.
Conduct periodic checks of client belongings, when necessary, to ensure prohibited items are not brought onto the premises.
Maintain a clean and sanitary environment by following universal precautions, including safely cleaning and disposing of bodily fluids when required.
Ensure high standards of cleanliness throughout the facility, including the daily sanitation of high-touch surfaces such as door handles, railings, and restrooms in accordance with health and safety regulations.
Perform routine janitorial duties including sweeping, mopping, vacuuming, and maintaining cleanliness in shared areas such as kitchens, hallways, offices, and client dormitory/room spaces.
Transport clients to and from appointments or approved program-related activities as scheduled or authorized.
Assist with operational tasks such as picking up client medications and supporting client move-ins or move-outs as needed.
Perform additional duties assigned by leadership to support program operations and client care.
Conduct all duties in an ethical manner that reflects the organization’s core values of integrity, transparency, accountability, respect, and responsibility.
Demonstrate professionalism, openness, honesty, and accountability when interacting with colleagues, volunteers, donors, and all individuals engaged with Illumination Health + Home.
Prioritize the health and safety of both clients and staff at all times.

Maintain appropriate professional boundaries with clients and colleagues.
Demonstrate the ability to meet performance targets and effectively manage workloads in a fast-paced environment.
Communicate concerns, updates, and feedback to the Site Manager in a timely, tactful, and professional manner.

Minimum Qualifications/Preferred Experience
Required:- High School Diploma or GED equivalent.
- Proficiency in Microsoft Office (Outlook, Word, Sheets, Calendar).
- Valid CA Driver’s License.
- 2+ years working in homeless services preferred.
- Bilingual In English and Spanish.
