Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description: 
The Manager of Community Supports Housing and Case Management will be responsible for overseeing the daily operations of the Housing Services and Case Management at the Carnegie Hall Navigation Shelter. This role involves managing key program personnel. The manager will ensure the quality of their work in terms of both programmatic and contractual compliance as well as client-centered and ethical considerations. Responsibilities also include maintaining staff productivity, representing the program in community relations, administering corrective actions when program standards are not met, Director of CSS Housing and Case Management with various administrative tasks. These tasks may include conducting documentation audits, participating in engagements with contractors, assessing reporting metrics, training new staff, monitoring program and contractual benchmarks, and contributing to workflow and policy development.
The pay range for this role is $68,640 per annum. 
The schedule for this role is Monday - Friday 9:00am - 5:30pm. 


 

Responsibilities:

Supervision:

  • Supervise staff for daily tasks, case oversight, training, and growth.
  • Coordinate hiring process of new case management and housing staff.
  • Work with site managers on all client decisions including exits, extensions, intakes, and other case planning action steps
  • Coordinates orientation and ongoing in-service education for staff to improve skills and client centered care.
Client Care:
  • Provide case consultation and emergency management for case oversight.
  • Provide management support to assist case managers, housing navigators, program coordinators and clients in creating care plans to support client’s progress towards self-sufficiency and goals.
  • Assessing a client’s physical and mental wellness interventions such as sobriety, safety, self-harm contacts, employment, housing, increase in income, mental health and/ or reunification.
  • Oversee care coordination in partnership with the city of Santa Ana and leads for all programs to ensure client care, effective case planning, and positive outcomes in line with the mission of the organization and contractual outcomes.
Collaboration:
  • Attend the Santa Ana by Name meeting, interdisciplinary care team meetings, and collaborations to build relationships and partnerships.
  • Coordinate resource presentations for case managers to increase knowledge of community resources.
  • Establishing and maintaining effective relationships with stakeholders
Documentation:
  • Ensure accuracy of data for contract compliance, and other reporting needs.
  • Provide program numbers for contract and grant reports as needed.
  • Assist the program managers with contract compliance as requested.
  • Maintain accurate records of client interactions, care and housing plans, case management and housing services using the Electric Health Record (HER) system.

Preferred Experience/ Minimum Qualifications 
Required:
  • Bachelor’s degree in social services, Healthcare or related field; or equivalent combination of training and experience.
  • Minimum 2 years of recent progressively responsible administrative management/supervisory experience.
  • Homeless services, case management, mental health, permanent supportive housing experience
  • Current valid CA driver's license
  • Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions.
  • At least 2 years of experience in the Social Services field.
Preferred:
  • Master’s degree in social services or related field
  • Bilingual in English and Spanish.
  • Proficiency in Microsoft (Outlook, Share Point, Excel, Teams and Calendar).
  • Ability to use databases such as HMIS
