Program Coordinator

Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
The Program Coordinator will receive, assign and keep track of all CSS Housing referrals for Inland
Empire, Orange, and Los Angeles Counties. Additionally, this position is tasked with receiving referrals for Medical Accompaniment and scheduling, assigning, and accepting/denying services. The pay range for this position is $26-28 per hour, depending on experience.
Responsibilities:
- Reviewing incoming CalAIM Community Supportive Services referrals preferred under 2 business days.
- Assessing if additional supporting documents are needed.
- Calling referrer contact if issues are presented with referrals.
- Inputting Client Personal Health Information (PHI) into various Health Network Portals and KIPU.
- Upon referral review, note all questions that may need to be answered by referral contact in order to ensure the client referred meets admission criteria.
- Utilize Google Spreadsheet/Microsoft Excel to create necessary data tracking information.
- Responsible for managing [email protected] email.
- Audit Community Supports Housing Navigation client profiles to ensure compliance with contractual obligations and internal workflow by utilizing the agency electronic health record and supporting tools.

- Implementing programs and policies designed to maximize the efficiency of the program and ensure that all client needs are met.
- Must display full understanding of the problems of client flow and scheduling, in order to improve the efficiency of program operations through performance of duties.
- Must demonstrate the ability to motivate staff, grasp new skills/procedures and foster a positive attitude toward changes.
- Must be an asset to the organization and a positive influence on co-workers, while displaying respect toward clients and others in the workplace

- Statistics and Data Reports
- Tally and collect all data and statistics on CS referrals, CS program, program utilization, and operation analytics. Provide reports as requested.
- Program and workflow development
Minimum Qualifications:
- High School Diploma or equivalent.
- Valid CA Driver's License and eligibility for company vehicle insurance
- Excellent organization and time management skills.
- 2 years of relevant experience
- Customer and client management.
- Strong self-management skills.
- Strong computer skills including the ability to prepare correspondence and reports using Google Workspace Applications and Microsoft Office Suite Programs.
